All of the following documents must be received by our office before processing your visa to Brazil.

VIETNAM VISA REQUIREMENTS:

• Order Form, thoroughly completed
• Your actual passport with at least 1 blank visa-designated pages.
• One passport-size photograph
• One copy of this Vietnamese visa application form, completed and signed
• A letter from a company is required for a Business visa.
• Payment: A company or personal check or money order made payable to "Travel Visa Express" (see table below for amounts). To pay for your Brazil visa by credit card, please download the credit card authorization form. Include the form in the package that you mail to us.

Business: A letter (on company letterhead) from Sponsoring Organization indicating the nature of applicant's business, probable duration of stay, places and organizations to be visited incorporating therein a guarantee to meet maintenance expenses, etc. should accompany the application.

Adoption Information

1. Application for adoption (Vietnamese Form No. 03TP/HT-NN)
2. Commitment of periodical report (Vietnamese Form No.04 TP/HT-NN)
3. Birth Certificates (husband and wife)
4. Marriage Certificate
5. Copies of passports (husband and wife)
6. Papers issued by the relevant American authorities and organizations in less than 6 months, including:
6.1 Police clearance; child, drug and alcoholic abuse clearance (husband and wife)
6.2 Home study (done by a licensed American Adoption Agency in the Applicants’ State)
6.3 Notice of Favorable Determination concerning Application for Advance Processing of
Orphan Petition – USA Form I 171-H)
7. Medical Reports (husband and wife) done in less than 6 months, certifying that the applicant is physically and mentally healthy, with no mental or communicative disease and is able to parent an adoptive child.
8. Employment and Income Verification, done in less than 6 months, certifying the applicant’s working situation and annual income with financial ability to care for an adoptive child.
9. Letter to the People’s Committee expressing the applicant’s desire to adopt and parent a child and commitment to preserve the adoptive child’s original culture.
10. Copy of birth certificate of the adoptive child.
11. Letter of Consent by parents of potential adoptive child freely agreeing to the foreign adoption of their child.
11.1 In case the child does not have parents and guardians, a letter of consent by the direct caretaker of the child to the foreign adoption is required.
11.2 In case the child is living in an orphanage facility, a letter of consent to such foreign adoption by the director of such facility is required.
11.3 In case the child is from nine to fifteen years old, apart from the letter of consent above, a letter of consent by the child is required.
11.4 In case the child who was left at birth at a health care facility was applied for foreign adoption, a letter of content by the director of the facility and a minute recorded the abandonment condition of the child are required.

We cannot guarantee that a visa will be issued as the Consulate may refuse to grant entry/exit visa for any reason.
