- India Disclaimer Form. Complete, print and sign the India Disclaimer tement.
- Letter of invitation from host or confirmed hotel reservation, including a copy of the host's passport.
- Passport Information Page Copy. Have a clear and legible copy of the information page. You will need to send this in addition to your actual passport.

- Invitation. A letter of invitation from the company to be visited in India. The letter must be on company letterhead, signed by a representative of the company, and must include the applicant's name and the name, address, telephone number and email for a contact person at the overseas company. Faxed or scanned copies are accepted.
- Letter of Incorporation From Inviting Company
- Hotel Reservations. Copy of confirmed hotel reservations.
- Former Indian Nationals. For those who have renounced Indian citizenship, one of the following must be submitted as proof: a Certificate of Renunciation issued by a Consulate of India or an Indian Passport that contains a stamp that specifically renounces citizenship.
